Family Note: Certain services provided by the USU Sorenson Legacy Foundation Center for Clinical Excellence may be eligible for scholarship funding, including: Individual mental health therapy for the scholarship student Speech and hearing services Applied Behavior Analysis (ABA) Physical therapy when specifically related to the student’s disability or special needs; general rehabilitation services are not eligible ASSERT Program: ASSERT is not an approved private school and may only be used as a supplemental program. It cannot serve as a scholarship student’s primary private school placement unless the program receives private school approval from USBE. Homeschool scholarship students: May participate beginning at age 5. Private school scholarship students ages 3–5: May participate in ASSERT in addition to their primary enrollment at an approved private school. Up to 3 Program: This program serves children through age 3. Because scholarship eligibility for private school students begins at age 3, the program may only be eligible for a scholarship student who is age 3 and enrolled in an approved private school. Up to 3 must be supplemental to the student’s primary private school enrollment and cannot serve as the student’s primary school placement.
